In accordance with the provisions of Section 10, Article IX of the AMSA Bylaws, I hereby designate _________________________________________ as my Proxy at the AMSA National Environmental Policy Forum & 28th Annual Meeting being held May 16-20, 1998 at The Washington Marriott in Washington, DC.

If no name has been designated in the above space, my signed statement designates the members of the AMSA Board of Directors to act on my behalf in all matters coming before the meeting.

In the event that I am able to attend the meeting, this signed Proxy statement will become null and void at such time.
