AMSA's Financing & Management Committee is conducting this annual service charge survey to update the AMSA Index for 1997. If your agency has participated in the past surveys (conducted in 1992 through 1996), your responses to those surveys are shown on the survey form along with the names of the person(s) who provided the information. Note that the 1992 survey was the initial survey and asked for historical information from 1985 through 1992. The 1993 through 1996 surveys asked only for that year's data. We hope this information will be useful to you in preparing your response to this survey. If you determine that historical information is incorrect or misrepresented, please feel free to modify these data directly on the survey form. Changes will be reflected in this year's AMSA Index summary.
In order to ensure consistency, this survey form should ideally be completed by the same individual who submitted information for your agency in the past (refer to attached survey form for the names of your agency's 1992-96 respondents).
