AMSA member agencies who will not be in attendance at General Session II of the Winter Conference in San Antonio, Texas on Thursday, February 5, 1998 are requested to sign this proxy form and return it via fax no later than 12:00 noon on January 30, 1997. Proxy Statement
In accordance with the provisions of Section 10, Article IX of the AMSA Bylaws, I hereby designate
as my Proxy at the 1998 AMSA Winter Conference in San Antonio, Texas.
If no name has been designated in the above space, my signed statement designates the members of the AMSA Board of Directors to act on my behalf in all matters coming before the meeting.
In the event that I am able to attend the meeting, this signed Proxy statement will become null and void at such time.
